How the documents are stored and how to retrieve them
- All documents are stored on multiple harddisks, DVDs, and other storage media for redundancy.
- The storage media is disconnected from the Internet, making the stored documents more secure.
- If you need to retrieve your electronic medical record, please use the Contact Us form, and we will get back to you with a Release of Information form.
- After we get back the signed form, we will inform you how to safely download your copy of the requested electronic medical record.
- The requests are processed as soon as possible, no more than 30 days from the date of the payment.
